Responsive to this request, please find attached a list of formal assessments used to gather relevant functional, developmental, and academic information. Assessment measures are considered on a case-by-case basis with qualified examiners determining which assessments to use based on the Individualized Education Program (IEP) team discussion during an initial or reevaluation IEP meeting. Additional informal tools, measures, and questionnaires may also be used as appropriate.
