Requested Information
I request copies of existing records concerning HCPSS Solicitation #075.25.B7 (School Based Mental Health Services), listed below, in electronic format.
-Records showing how HCPSS ensures vendor FERPA compliance, including training materials and training-verification records, monitoring or audit findings, and any corrective actions.
-Documentation of the student data exchanged in both directions between HCPSS and vendors (referrals, consent information, demographics, session attendance, progress information shared with school staff), including field lists or system-access documentation, and the authority relied on for HCPSS’s disclosures.
-Documentation identifying separately: (a) program records HCPSS receives or maintains, including the session-attendance data and weekly logs of students seen referenced in RFP §§ 3.2.B.3.c.2 and 3.3.4; (b) vendor-held records maintained on HCPSS’s behalf; (c) vendor records HCPSS can inspect or retrieve; and (d) categories excluded from HCPSS access. Include storage, retention, deletion, and parent-access procedures for program records.
-The blank consent and authorization forms in actual use under this program: parental consent to treatment, education-record disclosure authorization, provider-to-school release of information, and billing consent, with effective dates where documented.
-Documentation of the most recent completed backup of program data, showing date, scope, and responsible party, whether performed by HCPSS or by vendors, and the most recent export or transfer of program data to HCPSS.
-The procedure and place-of-service codes used by vendors under this program: any billing instructions or code lists issued by HCPSS, and aggregate or redacted records showing codes actually used, including vendor Medicaid or private-insurer billing to the extent held by HCPSS or on its behalf.
-The policies, forms, and workflow documents governing how students are referred for initial treatment, including eligibility criteria, authorized referral sources, and the steps from referral to first service.
