1)I am requesting Disciplinary Referral Forms for the current school year (SY25-26 (August 2025-present date, 4/29/2025) for Triadelphia Ridge Elementary, Fulton Elementary, Hanover Hills Elementary, and Thunder Hill Elementary. Please include:

• Date of Incident
• Staff Member / Referrer
• MSDE Category
• Behavior-related offense / Violation
• Assigned intervention and consequence, including no consequence (Disciplinary Action)

Please provide data on all office referrals that have been submitted, and not merely ones that resulted in a consequence.

2)I'm requesting reports of student, staff, or teacher injuries occurring at Triadelphia Elementary School for the current year (August 2025-the current date (4/29/25)). This request includes teacher/staff visits to Concentra or other medical clinics. I am also requesting the same for students. If allowed under law, I am requesting the cause, nature, and extent of the injury.

Responsive to your second request, in regard to staff injuries, worker’s compensation data on TRES staff who sought medical treatment (regardless of location where treatment was sought) for incidents that involved students for the current school year 2025-2026 YTD total five (5). Under the Family Educational Rights and Privacy Act of 1974 (FERPA), which restricts access to student records, and MPIA GP § 4-313, which prevents disclosure of school district records pertaining to individual students, as well as both under MPIA GP § 4-311, which prevents disclosure of personnel records of an individual, and MPIA GP § 4-329 (b), which prevents disclosure of medical or psychological information about an individual person, we are unable to disclose further details regarding the cause, nature and extent of injuries.
Additionally, in regard to student injuries, no data indicates the total number of students assaulted (victims) given one incident may involve more than one victim. Existing data on counts of student conduct violations for “attack on student” at TRES for the current school year 2025-2026 YTD total three (3).

Responsive to your first request, please find attached data from an available summary report on office disciplinary reports for the current school year 2025-2026 YTD from the elementary schools specified. Please note, this report only reflects those referrals for which a student violation was identified. No existing report identifies those office referrals submitted but ultimately not resulting in consequence as those are returned to the staff member without tracking. HCPSS must protect against the potential disclosures of personally identifiable information about students as required by both the Family Educational Rights and Privacy Act of 1974 (FERPA), which restricts access to student records, and MPIA GP § 4-313, which prevents disclosure of school district records pertaining to individual students. In accordance, given this report contains information on an incident-specific level about individual student discipline, the inclusion of incident date for an elementary school has the potential to reveal educational records
given the connection to a specific staff member/classroom. As such, we have included this data point on a separate sheet. Please note, there could be multiple violations of the code of conduct represented in a single student incident/referral, therefore entries represent each violation. For example, TRES has 22 total lines (violations) in the attached chart, however this represents 16 unique incidents as noted in the totals box. HCPSS has made substantial efforts to ensure the accuracy of this information; however, data collection and reporting is consistently subject to change. Please be mindful when comparing data as many variances can occur between reports based on date pulled, parameters used, categories included, etc.
