Please provide records indicating if any documents or series of documents in the care and control of HCPSS have been lost to fires, floods, earthquakes, theft, or any other unexpected event that has resulted in the loss of such documents. Also, provide any records of documents that were expected or required to be maintained but were unable to be found for a reason that is unexplained.

Responsive to this request, HCPSS Continuous Improvement & Records Management staff indicate they are not aware of loss of records related to unexpected events, nor do they maintain records of documents that were unable to be found for reasons unexplained. Additional time and clarification would be needed to continue response.
