Please include all nursing contracts for Private Duty Nurses ("PDN") or other nursing services provided by Aadvance Home Health Services, LLC to any and all HCPSS students. Please include the contracts and any information showing how nursing services are paid for during school hours for the HCPSS student(s) served by Aadvance Home Health during school hours by HCPSS.

I understand this request would include personally identifiable health information and completely accept and understand all necessary redactions to remove anything identifiable about the HCPSS student(s) involved.
