HCPSS MPIA Request #2018-362

Requested Information
Regarding the early kindergarten admission forms "Developmental checklist A - Preschool Teacher/Other Non-Related Adult" and "Developmental Checklist B – Parent/Guardian" under the Assessment items section, the four columns of choices under "circle one."

The forms WERE located at http://www.hcpss.org/f/schools/early-admission/early-admin-k-dev-b.pdf and http://www.hcpss.org/f/schools/early-admission/early-admin-k-dev-b.pdf

== Request 1 ==
Specifically, what definition should be used when responding to these choices:

Always
Usually
Sometimes
Never

No definitions for those terms are presented on the form, and two of the choices ("Always" and "Never") seem literally impossible truthful choices for a child.

== Request 2 ==
What timeframe should be considered when responding to each question, or the entire form as a whole? For example, should the child's behavior over the last month be considered? Last six months? Last 12 months? Entire lifespan?

No instruction is provided on the form detailing the timeframe in which the respondent should consider the child's behavior.
Status

Responsive to this request, the HCPSS Office of Early Childhood Programs indicates there are no existing records containing parameters for completing the developmental checklist other than the instructions contained on the form itself. Please feel free to follow-up with that office with further questions regarding the use of forms in the early kindergarten admissions process.
