Responsive to this request, please find attached requested data from an available summary report on office disciplinary referrals for the current school year from Howard High School. Date received/returned, administrator processing the referral (unless they were also the staff member completing the referral), and previous interventions are not recorded as part of the available summary information per incident. Please note, this report only reflects those referrals for which a student violation was identified. No existing report identifies those office referrals submitted but ultimately not resulting in consequence as those are returned to the staff member without tracking. Additionally, there could be multiple violations of the code of conduct represented in a single student incident/referral, therefore entries on sheet one represent each violation.
